  Has anyone seen this on an windows 2003 Server?

   

  Specifically am seeing this on a Windows 2003 R2 with SP2.

   

  This KB is exactly what I am seeing and I do have a homepage set in the 
Favorites and URLs gpo.  But the fix references Windows 200 not 2003

(SP1) on a Windows 2000-based computer that is running Terminal Services, if 
the "Important URLs & Favorites and Links" Windows 2000 Group Policy is 
enabled, Index.dat files may not be deleted on the server when a user logs off 
from a Windows 2000 Terminal Services client. This problem may occur even 
though the "Delete cached roaming profiles" group policy is enabled. This 
problem does not occur in Microsoft Internet Explorer versions 5.0 and 5.01 or 
if the "Important URLs & Favorites and Links" Windows 2000 Group Policy is not
