There is a KB article about this. The 'Cliff Notes' answer is: The user's Temporary Internet Files folder has grown too large (10% of a drive by default). Dump the Temporary Internet Files and reduce the size and you will be golden. As you can imagine - the ica file is being downloaded but the darn OS can't locate it and return it to the ICA Client fast enough.
