[THIN] Password Manager

I've got an issue with Password Manager 4. I've an application which is opening
up and should prompt the first time the user for the username and password.
This was working fine.
Now the application opens and seems to ignore the Password Manager agent. I can
see that the user is having the ssoShell.exe running for their session. This is
now happening for everyone. I recreated a new copy of the app, application
definition and user configuration but again this does not work.
Any ideas?
