Turns out to have been the SMS 2K3 agent that was installed.  There is a
patch for the errors I was receiving and after I removed it from the
affected servers, BAM!, no more slow/non-existent logoffs.

Hi Chad,

Something's snuck in on you. 

Check the processes running inside the published application session. My
bet is you'll find an extra process that isn't dying when the published
app closes. That would keep the blank session open.

All of the sudden, users logging off of published apps. the app. closes,
but the connection center still show a connection to the server.  Choose
logoff, nothing happens.  CMC shows no connection to the server. 

I tried making ICA connection to my published desktops, I go to logoff,
I get a blank blue screen, no "saving your settings"., nothing.  Just
sits there, until, what I believe to be my 10 minute disconnect timeout.
