[THIN] Ica disconnections

Hi list,
we've just installed a new farm, new servers as well.
Ps 4.5, 2003 MUI R2
also, we set up one web interface to connect to.
for some reason, we are having some disconnections randomly, and after this
occurs user cannot reconnect because the server doesn't accept more than
instance.
We are a bit confused about this because we have no disconnections either a
published app. or ICA/RDP protocols or user session in AD.
Anybody could point me in the correct way?
