Ok, I have tried the listed stuff below, but it is still not working.  Any 
other thoughts on what might be preventing this from functioning?

I noticed that no one replied to your email.
We are essentially doing the same as you are. And we DO have pass through 
authentication working.

First things first, make sure you have the latest client installed. We 
spent a long time playing around with version 7. With this version you did 
need to mess around with the appsrv.ini etc.. But we have not found that 
on version 8...

So assuming that you have version 8 here is a sample .ICA file

Simply generate an ICA from within citrix and then ensure certain lines 
are included such as USELOCAL..
