The shell is correct but the user still gets the error message when I do the
notepad

Sounds like either security or a policy within the default profile.

To test if the Shell (desktop) can be run, you can exploit a little flaw in
Terminal Server Published applications.
login to your Outlook published application, open a new note  type in \\<the
local WTS servers name >  e.g. \\TestServer so that it turns into a hyper
link (turns blue and is underlined) then click on the hyperlink.

This will evoke explorer.exe and bring up a full desktop.  If it does not,
then explore is either secured dis-allowing execute access or there is a
policy applied somewhere denying the user access to the shell (desktop).  If
it brings up the desktop then the problem maybe within the login
initialization. 

Is the shell set correctly in HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows
NT\CurrentVersion\Winlogon\Shell ?

ok.....so if he can run outlook, its not his profile as he will need that
regardless of whether he logs onto a desktop session or a published app.
I'm afraid I'm out of ideas......what does your usrlogon and usrlogn2.cmd do
during the logon process...have you ammended them?

Ok, if my user runs outlook as a published app from the same server, he can
open outlook, but he still can't run a user desktop. I deleted his profile
and shutoff the logon script and still no go....

It certainly sounds like there is something in their profile that is not
accepted by NT 4.0, especially as you said you have a W: subst drive.....

Yes I do, but a user I am testing only logs on to my NT4 boxes , he never
logs on to windows 2000, his profile does not have the .tmp issue

Do you have any 2000 Terminal servers?  Has the ntuser.pol file in the
profile got lots of .tmp extensions?

All of my users who logon to my TSE 4 boxes are getting "can't access
folder. Path too long"
I was told that this was a w: drive not there problem in the past but today
I don't know.
My W: shares are there, their permissions are there.   What else can cause
this?
