We had the same issue with our Win2003 systems.  We had to apply the
following to the registry.

Windows Registry Editor Version 5.00

Hitting Control-F1 didn't bring up the task manager for me.  My pagefile is
fine too.  I ran process explorer from sysinternals and noticed lsass
tokens for each of my login attemps were hung.  I didn't want to kill the
handles though because Im not sure if that would crash the server.  I'll
reboot the servers tonight to see if it clears it up.

 I had a server do this yesterday after it had been upgraded from XPe to PS
 4.0. You could hit the control-F1 in the session, choose Task Manager,
 start a new task, and run explorer.exe, and the desktop appeared, but the
 real problem was that, for some reason, the pagefile settings were blank.
 Once I gave it valid pagefile settings and rebooted, all was well.

 I was logged in to my published desktop, I logged out fine.  I logged back
 in a few minutes later and the only thing that comes up is a black screen
 with the mouse cursor.  I have to log in as someone else to reset the
 connection.  This is happening on all 3 of my MFXP FR3 servers.  RDP or
 ICA.  I noticed that there are NO processes running for the account when
 it's on that blank desktop.

 Any idea's on what is causing this?  My servers are all Win2k sp3.
