I guess I answered my own question with a bit of research. For some reason,
this option is grayed out if there are no printers on the server using PS
driver. It doesn't have to be my printer, there just has to be one there
from any user. Very strange.

Has anyone experienced the problem in Adobe Acrobat Reader 6 where the
"Print As Image" option is grayed out? Actually, everything in the Advanced
button is grayed out.
