[TechAssist] Funai combo's - repeat failure of D615. Component part no.

Can anyone give me the Mfr. part numbers for these Funai components?
I replaced D615 (2) times, (shorts back out after about 2 minutes. Don't
have a schematic on these!

Q606
Most of the tips I found indicated replacing all these components to keep
D615 from shorting out.
