You are trying to confuse us with facts but it is quite elementary, my dear 
Watson. 

Fundamentally, it is an intermittent problem so therefore a loose connection. 
It is isolated to the #4 plug so it is either a bad spark plug or spark plug 
wire, both which could be determined visually with a little effort.
