Perhaps Sanjeev has a point not well expressed... if I assume he means jobs
that are under a gun, mgmt with "blinders" on regarding process, methods &
tools and who have misconceptions.
Model criteria & layout practices assumed awesome. EE101 ideas settling
into the memory hole. What could go wrong?
That is, where light isn't allowed to shine. A black hole of sorts. Where
not much, or perhaps anything, it is suspected, can actually be contributed
because sub-optimality is built in.
I done been there, done that. To wit:

Didn't detect the gremlin hiding in plain sight: reflections nullify the
famous-fantabulous-unique methodology. Hspice obsession. 4 PhD on project.

Well, I did my job to show it, using that very methodology. What a hoot.

What could go wrong? answer: Everything. ...They're gone.
I did my heroic thing... apply basics, change much.
Didn't help them ulitmately but I self-learned a bunch... they didn't.
At some point, it's... meh, I'd rather take off my shoes, climb a tree and
learn to play the flute.
