If we follow the definitions, you would also need the impulse responses
(or its integral, the TDR response) of the two ports, while the other
port is properly terminated.

I am not sure you can do it unless you have some extreme situiation. From
the complex fft of h(t) you get one complex number per frequency, but you
have three unknown complex values (S11, S22, S21ñ2 if the DUT is reciprocal).
