Those itchies and hair loss could be allergies and/or a yeast  infection.  I 
had a dog that developed this when he was 2.  Took about  3 months to get him 
together again.  He got a change in diet, frequent  oatmeal bathing, an immune 
system strengthener supplement, with an occasional  benadryl for those 
itchies that were just too much to control.
 
Antibiotics are good if there is an infection.  If there is a foul  odor, it 
is more than likely yeast related, then you have to balance what the  
antibiotic is doing.  Been there!  Not fun!  But can be  done!
