I ran this case by my daughter, who has been working at veterinary  
hospitals for the last 11 years, and she feels that once you rule out this as 
an  
obsessive behavior that comes out of boredom,  it sounds as a progressive  
case of food allergies, with the starting of licking his paws and getting 
worse  from there.  They see a lot of animals who are allergic to chicken and 
corn  that come with these beginning symptoms. A change of diet to a different 
 protein seems to work, unless the itching and scratching has become  
chronic. When that happens Prednizone (given only during a short period of  
time) 
seems to work well with these dogs, they get a certain amount of relieve  
once they stop the itching cycle.  Of course, no one knows your dog better  
than you, but maybe this is something worth discussing with your vet.
She wants everyone to know that dogs on Prednizone and Phenobarbital MUST  
have water available at ALL times.

Here is  the history as I have been told:
Male dog, 1-1/2 years old.  This dog  licks his paws/chewing his feet, and 
about 5 weeks ago, he started licking his  rectum.  Owner noticed that his 
rectum was bright red.  After trying  her own things at home (I don't know 
what other than Vaseline), she took him  to the vet.  Vet (who sees many GSDs) 
does not think it's fistulas, but  there is a "hole" there, or something.  
He changed dog's diet, on venison  now, which didn't make a difference. He 
prescribed an antibiotic (maybe  sulfasalazine), tacrolimus cream and oral 
prednisone.  Dog not getting  better.  Rectum still red, "hole" still there. 
Vet recommending biopsy  and thinking it's something autoimmune.

Here's what I know of dog's  history:
1.  Vaccinated including Lyme.
2.   Wormed.
3.  Likes to be outside.
4.  Chases his tail a  lot.
5.  The dog is still being crated every day 8:00-2:00 but runs in  the 
field "to get his anxieties out."

So, it looks like we have an  itchy dog, feet at least, and now red rectum 
with a hole or depression or  something. Can anyone help me help this woman?
