I had a puppy bitch with Vestibular as well. My vet examined her and
that was her diagnosis. At about 5w as well. Suddenly there it was. I
said she looked like the little puppy that did the RCA commercial. We
called her RCA for a couple of days, but it soon turned into Reeca
(sounds like Reesa) She is now 5 and doing great. It has never affected
her like at all. She does still have a slight head tilt.

Is the head slightly turned to one side vestibular? If it is, then a
puppy I bred has it and is now 7. As a 5 week old pup, his head was
partially turned to one side. I took him to the vet's and he had NO
IDEA what it was. We named him Twister, since I worked with him
therapeutically several times a day, gently "twisting" his head from
side to side--I thought it was a muscle problem. I gave him to my vet's
tech's cousin and he lives about a mile away from us. For about a year,
the problem seemed to correct itself, then it came back, but to a LESSER
degree. He now tips his head (almost imperceptibly) to one side.
Weird. And he's a lovely black and tan, almost a bi-color with an
exquisite temperament--evidently the tipping is not painful. And his
name? Landsend's Diamond Twister.
