Here is what we put on our tags:
Hi my name is Pepper, I am a black and tan German Shepherd.

Connecting Flight #: (if there is one)
Final Flight #:
Final note we always include:
"Please do not open my crate under any circumstances as I could become
frightened and escape.
We included frozen buckets of water, a chew bone, thick blankets and a BUCKLE
collar with a complete set of tags. I prefer name plates verses tags, but that
is just me.
I cable tie the front door and use a bungee cord along the front also.
I carry two copies of a the rabies certificated and a recent photo of the dog
too. My crates are also covered in stickers and I usually include a couple of
weirdo bumper stickers on the side so the crate is easily identifiable to me
and anyone else.
