The only time I ever lost one was went I spayed her at the time of a C Section, she bled out and there was nothing anyone could do about it. Apparently not that rare of an occurrence. Wise and sad words. The uterine tissue is still engorged w/ blood and try as the vets might to get everything as tight as possible, as the swelling diminishes, the area where the (end of the) uterus was "tied off" may become "loose" and the bitch will bleed internally. IF you HAVE have to spay simultaneously, for goodness sakes, leave her at the vet for 48 hours minimum, so if God forbid something goes wrong they have a chance to go in and save her.
