A dog the size of a GSD should have no need to have their anal glands  
expressed as a routine procedure. They are (in general) able to do this on 
their 
 own with just the normal straining of a bowel movement. 
 
We deal with lots of little dogs in the business, that's a whole different  
story.  And you will be surprised to know how many owners have no idea how  
to express their dogs anal glands, or how to recognize if that's the  
problem.
 
We had a couple of GSD fosters that had PF, and (again, in general)  they 
were just not healthy. That's when we first became aware of this illness,  
and, indeed, antibiotics alone will not solve this problem.  There were  diet 
changes, constant rinsing and cleaning, researching what meds work and  what 
made things worse....a real nightmare. 
 
First obstacle is to get the right diagnosis.

That is  true there is a hugh difference. PF can not be cured through 
antibiotics or  surgery.  Cyclosporine is the major treatment along with change 
in diet,  but the problem can only be put in remission, sometimes weeks, 
sometimes  months, some even years.  It is thought to be genetic, and there is 
even  a genetic test for it. Usually triggered by stress or vaccinosis. There
