Ugghh....I am up to over 15 pages of instructions on everything from  puppy 
proofing a home/ when-how to feed/ housebreaking/ selecting a  
vet-trainer-puppy kindergarten/ containment and on and on and on....
 
I also send duplicate copies of immunization/worming records and suggest  
topics to discuss w/ the vet (one for the owners, one for the new vet;  
immunization schedule, heartworm protection, spay/neuter, flea/tick protection, 
 
microchip) AND a copy of the puppy's health cert from MY vet.  On their  copy 
of the 
immunization record, I include the little sticky labels from the  vaccine 
vials (vets are sometimes "dubious" about breeders who do their own  shots).
 
I send a recommended reading list by category and a list of recommended  
catalogs and web sites ( PC, AKC, Pet Edge, Revival, KV Vet, Cherrybrook,  
etc.)  
I actually include item #s for things to keep on hand in a  doggy medicine 
cabinet.
 
I have duplicate copies of my contract, one for me, one for them.   They get 
a 4 generation pedigree w/ nice photos of the parents.
 
I don't send the papers until after the puppy is checked by their  vet.
 
I do use the Purina pro Plan puppy kits.  There's a lot of useful info  in 
there.
