This is exactly the same protocol we used on a preemie. It worked great.

Royal Canin mother and baby mousse added to the milk. In a few days a
little bit of soaked RC Maxi starter added to that. Then take away the milk,
a few days later take away the mousse, gradually don't soak the starter as
long.
I would take away the yogurt completely and use either bene bac or forti
flora.
