It used to  be an option but according to the receptionist at a vets office I 
was at it's  mandatory.  Since Cherish will be microchipped when she's xrayed 
in a few  weeks it's not an issue I'm just curious.

No, it's not a rule.  The form is on the OFA web-site and if you  put "n/a", 
your dog's report comes back w/ N-PI for "no permanent  identification" at the
