[ SHOWGSD-L ] Health Merit excellent

Could anyone tell me what I have to send for the Health Merit Excellent?
I know where to send it but what should I send. I looked in the 2002 Red Book
but it really doesn't say what needs to be sent. I cannot find my 2003 Red
Book and I looked in the Roster and it doesn't mention it at all.
