[ SHOWGSD-L ] Futurity litter nonmination

I just printed the 2005 Futurity Nomination. It's ok for my one litter that
was whelped on Sept 22 04 But my second litter was whelped on Oct 23 2004.
The Litter form does not cover this litter. Where do I get the form for my
second litter??
