Found that to be true w/ cardiac too...there is a space on the form where
you can enter the chip # (or tattoo I guess) and the vet initials that they
checked, but not mandatory...although...I wouldn't keep a dog much let
alone breed one w/ a heart issue, and I know at 6 weeks when they are vet
checked, so I never stressed too much about the OFA and the various forms.

I see now where it says that thank you. I stand corrected. That concerns
me now too. Only difference then is the vet putting themselves on the line
saying yes I certify this is the dog stated above to the best of their
knowledge. You would think they would want it required. That to me is very
disturbing now knowing this.

The OFA does not require, take a look at the submission form....maybe your
vet? You don't even have to have a name. I prelime'd a bitch before she
was even registered.

Unless it has recently changed a dog does not have to be permanently id'd
to
be ofa certified . That is only required if you want akc to acknowledge it
on the pedigree . Granted i don't keep up with the changes since i do not
any more
