It is of course possible to have "formerly of,"  but without more
specific wording, I would treat it like a place of birth, which is not
necessarily a place of residence.

"Formerly of Hillston?"  No.  It could be the family's cemetery.
People go to retirement villages and hostels where they can find a
vacancy.  When they die, they are buried where their family lives.
