I am not sure, but assuming you didn't change the $user_auth_src option, a kind
of a builtin control panel/webmail may be getting into authentication process.
I would use Wireshark to make sure, that the browser actually delivers correct
credentials to the web server, and checked the $_SERVER PHP array to contain
these in PHP_AUTH_USER and PHP_AUTH_PW.
