RAC CONC-SM TNS FAIL message in CCM log file

Hi,
I am running a single-instance 11.5.9/9.2.0.6 environment. The following
message is appearing in the CCM log file:
CONC-SM TNS FAIL

The managers are running fine and there is no other error in the log
file. What is the best way to diagnose the root cause of this message?
