At this moment, I'm uncertain if Amis will support BSO content but I plan on
working with them to make it happen at some point in the future.

I think Amis (pronounced ahmee as in the French plural for friend and not
Amos as in Famous Amos the cookie) runs on Vista.  It's open source, GPL,
free software and its authors announced that they will have a new beta with
a fairly major upgrade within a few days.  You can find the executable via a
google search and the source code (nearly a year old so it's probably worth
waiting for the 3.0 source to be posted) on SourceForge.

I unfortunately have to read the book as HTML because there is no vista
player for bookshare books that is downloadable from bookshare that I am
aware of.

The positioning of the footnotes and other elements in the text should be
controlled by your DAISY player and not through your source file.  That's a
personal preference.
