I think that your second idea would be the best. You said that you share a
template, do you mean a excel file template?
If it is the case, after the templates have been filled, how do you collect
the information that people has added on those templates?
Excel Online Forms is able to add the information added on the forms
automatically in an excel sheet. However, Microsoft is discontinuing
Microsoft Forms online and wants to push everybody to use Microsoft Forms.
When you said that you have tried Microsoft Forms and that it was not
successful, do you mean the accessibility or just that the Microsoft Forms
was not a good solution for your purpose?

Are the PDF files in a standard layout or does the layout change?
If the layout is standard, then you could take the PDFs, import them into
Excel, clean them up and convert the whole process to a macro.
Another workaround I have used is to issue a template to people, They fill
that template and that's it. Instead of PDF files, you get excel sheets
which you read into the system.
I have tried using Microsoft forms without much success but that form design
is a onetime activity so I would not rule them out.
