For the benefit of other Phorm users, after some troubleshooting by e-mail, I sent a working copy of Phorm, configured to send the three attachments. Björn then copied my version of phorm.php and fileattach.php3 over his and everything worked. If anyone else has a similar problem, please ensure that you have the latest copies of the program.
