Thanks, this cleared up quite a bit. But each time I fix something, something 
else goes wrong. 
Now, I am getting the error below: 
ERROR 
000 - Catastrophic error: Unable to read Phorm function library 
../lib/functions.php3. 
090 - There has been an error, and your information has not been sent. 
This is how my files are set up: 
site: 
form.html 
form folder: ack.html and emaillog.txt 
phorm folder: lib folder (functions.php3), phorm.php, phormconfig.php3 

I have set the permissions, I believe, correctly. 

Sorry this is not going well, but I appreciate your help.
