I responded to this by email, but I'll put it here for posterity (and I'll add 
something to the README) 

What's absolutely required is phorm.php and the lib directory. The files 
directory is highly recommended. I also recommend the plugins directory.

It's a tool for installing and configuring Phorm. If you have problems with 
the installer, email me and I'll send you the distro. Then just point your 
browser to phormation.php
