When I use $PHORM_ACK this problem does not occur. 

However, due to various path problems, I think I have to use $PHORM_RDIRECT. 
When I use $PHORM_RDIRECT the problem I described does occur. 

I know the "Cannot modify header..." message is a PHP message.
