[phorm] Ok I have Registered Phorm3.5.1

Hello;
Ok I have Registered Phorm3.5.1, now where do I get fileattach.php? I have
configured PHORM_ALERTTO to send me alerts from Phorm.Processor@xxxxxxxxxxxxx
Internal Alerts:
010 - Unable to open file attachment module. Responder attachment not sent.
I'm not planning to send an attachment for now, but I will in the future.
If I can't get a copy of fileattach.php from you where do I get it? It's
not in phorm.zip which I downloaded from:
http://www.phorm.com/phorm.php3
The link to get a registered copy of Phorm3.0 at:
http://www.phorm.com/register/regack(_My-registration-key_).html
is broken. Check out http://www.phorm.com/register/phorm.php3
I get these 3 errors in this page

Apparently there must be some other way to get a registered version of
Phorm3.5.1
