Jeffery, who owns this in-house developed stored procedure? Did they need a
direct grant on a SYS owned object to perform the task? If so, SYS issued
direct grants can be lost during upgrades and migrations requiring the object
privileges be re-issued. Maybe this is your issue. If you had posted the
Oracle error returned on the GRANT it would have been helpful. A portion of
the code might also be needed if this is not the issue.

I have a procedure that a system admin uses to create an oracle account and
grant create session privilege and a role. This was working fine in 11G. We
have now upgraded to 12c. The grant of the create session privilege is failing.
