But, glassfish actually *does* start up. It's just the Oracle doesn't know
it:
crsctl status res glassfish
NAME=glassfish
TYPE=ora.local_resource.type
...
I *think* the most relevant error is "Failed to initialize agent, could
not get type name..", but I have no idea what it means, and searching
MOS and Google has not been successful, thous far.

Earlier you specified type generic_application. Right? I wonder why it changed
to ora.local_resource.type. If it's generic_application type, maybe you're
hitting Bug 1382560. See
generic_application resource does not start if CHM is not online (Doc ID
1968575.1)
Just try changing it to cluster_resource.
By the way, the error message comes from scriptagent.bin and seems related to
__CLSAGFW_TYPE_NAME.
