Replacing RAC node Hardware

Need insight from Experts that if I had one 5-node RAC setup where 5th Node is 
having hardware issue and need to be replaced with new Hardware. What is best 
recommended approach. Is it only been possible with removing and then adding 
the node or we can take image backup and load into new hardware or so
