Jo -

Thanks.  It appears there was different versions of opatch in the OMS, Agent 
and DB homes and I must have used a later version when applying the last patch 
to my OMS home.  I now installed the latest version of opatch outside of any of 
the Oracle homes so that I use the same version for all homes on this server.
