The problem is with the disk channel - see the below - ora_disk_3 is trying to
write to the root file system:

Your previous log shows all backups are going to SBT, not disk. So either your
default Rman device type has changed from sbt to disk, or you are running a
script that is using disk channels.

the format of the media manager save set name, not the root file system on
