I don't have a standard form, but we use some tables, an APEX front-end and
different methods to collect/insert the data.
For us this is sufficient 😉

I'm looking to create a map of everything that touches our production
Oracle databases. The document needs to serve at least two purposes.
First, I need to know the Oracle-specific details of the connection (e.g.
platform, Oracle client type, version) for upgrades and troubleshooting.
Second, management wants to document data flow in and out of the DBs. This
might be a combined document (thinking about future maintenance), but it
doesn't have to be.
I whipped up a sad and confusing Visio doc of all Oracle clients when I
last upgraded our DBs. As I'm now looking to recreate the doc, it seems
there should already be some "standard" map/method/template for this type
of info.
Anyone?
