You can look at V$DATABASE_INCARNATION, but it doesn't track all history and
there of course are things as human you can do that would appear here too.

Subject: GENERATE REPORT FOR SWITCH OVER/FAIL OVER FOR DATA GUARD
We have Oracle 126.96.36.199 Active Data Guard. Upper level management recently
requested a report that will show switch overs and failovers, date and time,
from primary to secondary instance name, etc.
If the document exists that explains/shows how to accomplish this I have not
been able to locate it. The only thing I have come up with is getting
information from the dataguard log which is very cumbersome.
Does anyone have a more elegant solution?
