Which "instructions" are you referring to? in the README for 9119284 (PSU#3 patch for ORacle 10.2.0.4) after you run the opatch apply, the next thing is Post Installation INstructions 2.3.3 In that you load modified .sql files into the database. @catbundle.sql psu apply. That's the process that puts the info into registry$history, right? Even in the 2.3.4 of that readme, it says you must execute 2.3.3.1 steps... you have it in your original email.
