Primary Outage: ? Outage in exactly what regard? 

 

Was it hung waiting for an available online redolog group, or did it
actually come down?

 

What were the messages (if any)?

Did any background processes stop? (If so, which ones, and was there log
information?)

 

I'd include this in the "should not happen" bucket based on the preliminary
information given.

Thanks for the update. Log_archive_dest is not marked Mandatory. We also did
few times patching on Standby as well OS maintenance and taken it down and
that doesn't caused the outage but FRA filling on standby caused Primary
outage is still mysterious. So want to check as how this is related.

It can if the log_archive_dest that points to the standby is marked as
"Mandatory". Otherwise if the standby is down the primary will just continue
one and then RFS/FAL will fetch the missing logs when the standby is back
