Yes, that is an offset, but that doesn't mean that only offsets are used
in your TSTZ columns (if you have any).  If you're on 9.2, then you
don't need to worry about the patches unless you're using TSTZ/TSLTZ
columns in your app's schema/code somewhere, or plan to do so in the
future.  If you are using TSTZ/TSLTZ, then read all the notes, download
the patch and run the utlzuv2.sql script after manually adding the 9 new
timezones to your timezdif.csf file (see my previous posts for more
detail on this).  Yes, lots of us have dealt with this - please review
all the Metalink Notes and archives of this list and then let us know if
you still have any specific questions.
