Checklist for create a new database

Hi Listers,
Programmers from different kind of application asked the damagement to
create a new database.
Other programmers for a new small application will do the same.
I want to make a Checklist and/or Standard Operating Procedure (SOP) for all
programmers and damagement about when to create a new database or just use a
new schema in an existing database (Development and Production).
Do you have that Checklist or SOP ?
