I have this resolved. Ran node clean up, changed the order of the
utl_file entries, bounced the DB server, rechecked the APPLPTMP
variable, and then ran autoconfig on the DB/App Tier everything worked.
What is the significance of the APPLTMP variable, I understand APPLPTMP
is important, the APPLTMP and APPLPTMP are different?

Also you do not need to run autoconfig to refresh utl you just need to
restart your db.

Can you check whether both $APPLPTMP & $APPLTMP pointing to same dir? if
not,
change them to same value appropriately and also make sure this dir is
mentioned
as first entry in utl_file_dir init ora param
