In one incident that I have been analyzing, an invisible player is created in
the following scenario:

Player A kills Player B with a SSG.
1) Server sends Player A a msg to spawn Player B
2) Server sends Player A a msg that Player B was killed and also sends the
obituary msg.

Normally, Player A would be notified that Player B was killed before being
notified that Player B spawned.  This appears to be a server bug, though the
cause of the message order reversal is as yet undetermined.
