Location: Mount Sinai Hospital
Description: The Mount Sinai Hospital is hiring clinical pharmacotherapy
specialists to support Mount Sinai Transitions of Care projects. These
pharmacists will work with patient care teams to improve the quality of care
and outcomes for their patients.
Anticipated responsibilities will include, but
not be limited to:
* Providing patient education in self-management skills, including
self-injection, use of home infusion pumps, blood glucose monitoring,
therapeutic drug monitoring, inhaler user, and dietary, lifestyle, exercise,
and smoking cessation counseling.
* Conduct admission and discharge medication reconciliation
* Assist patient access to their discharge medications
* Work with the interdisciplinary teams to ensure adequate patient
education and engagement prior to discharge
* Consult with Case Managers and Utilization Coordinators to ensure
access to medications upon discharge
* Develop follow up plans for patients with high risk regimens
* Supervise pharmacy students involved with the medication
reconciliation process
Qualifications: Applicants must have a minimum of two years clinical
experience or a PGY1 residency, have an active (or be eligible for) NYS
pharmacy license. Preference will be given to bilingual applicants and
applicants with active Board Certification, PGY1 and 2 residency training, and
experience in transitions of care settings.
