Colleagues,

We are reaching out to other healthcare facilities in an effort to further
improve our Medication History process. Currently, our medication histories
are completed by a triage nurse who modifies a prepopulated list of medication
generated from the previous discharge medication reconciliation. The nurse
compares this with a medication list provided by the patient and an electronic
medication fill history (ie. Sure Scripts) upon admission to the Emergency
Department. How does your institution conduct Medication Reconciliations?

Who conducts the medication history?

* Triage nurse in the Emergency Department

* Floor nurse upon admission

* Admitting nursing service

* Pharmacist in the Emergency Department

* Clinical pharmacists rounding with the medical team

* Staff pharmacists

* Pharmacy residents

* Medical residents

* Medical interns

* Pharmacy students

* Medical students

* Pharmacy technicians


How is the medication history conducted?

* Modification of a list of medications from a recent
admission/discharge and compare it with a list obtained from the patient either
verbally or written

* Formulate the complete list from scratch with the patient either
verbally or from a written list provided by the patient
What information is used to complete the medication history?

* Patient's pharmacy fill history

* Further follow up with the patient's pharmacy and/or insurance company

* Patient's own medication list

* Verbal information from the patient

* Electronic medication fill history (ie. Sure Scripts, etc.)
When is the medication history performed?

* In the Emergency Department as soon as person presents regardless of
patent's mental status when able otherwise at some point within 24hrs. We do
about 80% of admissions.

* At some point within the first 24 hours of presentation

* Once the patient or another responsible party (ie. HCP) is available
to give accurate information regardless of timeframe
